When calling to schedule your first appointment, be sure to have your health plan information available.
We will need your contact, insurance, and medical history information before your first appointment.
There are policies and other information we will also need for you to review and sign before your first visit. To make this easier, NEW patients can download, print and fill out the forms before your scheduled appointment. This will allow you to check-in for your appointment with all of the necessary paperwork already completed which allows our staff to provide you with FASTER service.
We can also mail, email or fax the forms to you so you can fill them in by hand and return them to us. If you have any questions about these forms please feel free to contact the Apollo Care Coordination Team at (916) 236-5800.

It’s likely that your insurer may not cover some or all of the cost associated with your treatment. Contact your health insurance plan directly with questions about what’s covered and what your co-pay amounts are for different services. For amounts not covered by insurance, we offer several convenient ways to pay, including:
You may pay your bill by mail or phone. Please note that you may receive multiple bills for different portions of the same visit or medical procedure—such as from doctors, for diagnostic tests, and for services rendered at the facility location where your care is provided.
